What Is All-on-4?
All-on-4 is a dental implant technique designed to replace a full arch of teeth — upper, lower, or both — using just four implants per arch. Unlike traditional implant approaches that may require six to eight implants and extensive bone grafting, All-on-4 uses two straight anterior implants and two posterior implants tilted at up to 45 degrees.
The angled posterior implants are the key innovation. By tilting them, the implant engages more of the existing bone and avoids anatomical structures like the sinus cavities in the upper jaw and the nerve canal in the lower jaw. This means many patients who have been told they don’t have enough bone for conventional implants can still be treated with All-on-4.
A custom-made prosthetic arch — a full row of teeth in acrylic, composite, or zirconia — is then fixed to the four implants. The result is a set of teeth that are permanently attached, do not come out, and restore chewing function to near-normal levels.
How All-on-4 Treatment Works
1. Consultation and 3D Imaging
Dr Pham will examine your mouth, take dental X-rays, and capture a CBCT (3D cone beam) scan to assess your bone volume, density, and the position of nerves and sinuses. This imaging is essential for planning the exact angle and depth of each implant. You’ll receive a written treatment plan and cost estimate before any commitment.
2. Surgical Placement of Four Implants
On surgery day, any remaining teeth in the arch are extracted and the four implants are placed into the jaw bone according to the pre-surgical plan. The two posterior implants are angled to maximise bone engagement. The procedure is performed under local anaesthetic, with sedation options available for patients who are anxious.
3. Temporary Teeth Fitted Same Day
Where the implants achieve sufficient primary stability, a temporary fixed prosthesis can be attached on the same day as surgery. This means you leave the clinic with a full set of functioning teeth rather than waiting months with no teeth or wearing a removable denture. Dr Pham will advise whether same-day loading is appropriate based on your bone quality.
4. Healing and Permanent Prosthesis
Over the following three to six months, the implants integrate with the surrounding bone in a process called osseointegration. During this period you wear the temporary teeth and follow a soft-food diet for the first six to eight weeks. Once integration is confirmed, impressions are taken for your permanent prosthesis — a precisely milled arch that is screwed onto the implants for a secure, long-lasting fit.
Advantages Over Dentures
Fixed and secure. All-on-4 implants are screwed into your jaw bone — they do not move, click, or slip when you eat or speak. There is no adhesive paste, no soaking overnight, and no embarrassing moments.
Bone preservation. Dentures sit on the gum and do not stimulate the jaw bone, which causes ongoing bone resorption and changes to facial structure over time. Implants transfer chewing forces into the bone, helping to maintain its volume and density.
Eat normally. Denture wearers often avoid hard, crunchy, or sticky foods. With All-on-4, you can eat steak, apples, corn on the cob — foods that many denture wearers give up entirely.
No slipping or sore spots. Removable dentures can rub against the gum, causing painful ulcers. Because All-on-4 teeth are fixed, there is no movement against soft tissue.
Natural appearance. The prosthetic arch is custom-designed to match your facial proportions and desired tooth shade. The teeth emerge from a gum-coloured base that sits flush against your natural gum tissue, creating a natural-looking smile.
Treatment Timeline
Initial Consultation and Planning
Comprehensive examination, 3D imaging, and treatment planning. Dr Pham will discuss your options, explain the procedure, and provide a written estimate. This appointment typically takes 45 minutes to one hour.
Surgery Day
Extractions (if needed), placement of four implants, and fitting of temporary fixed teeth where possible. The surgical procedure typically takes two to three hours per arch under local anaesthetic with sedation available.
Temporary Teeth and Healing
You wear the temporary prosthesis while the implants integrate with the bone over three to six months. A soft diet is required for the first six to eight weeks. Regular follow-up appointments at our Parap clinic allow Dr Pham to monitor healing and adjust the temporary teeth if needed.
Permanent Prosthesis
Once osseointegration is confirmed, impressions are taken for your final prosthesis. The permanent arch is milled from durable material — typically zirconia or high-strength composite — and screwed onto the implants. This is the restoration you will wear long-term.
Are You a Candidate for All-on-4?
One of the advantages of the All-on-4 technique is that it works for many patients who have been told they are not candidates for conventional implants. The angled posterior implants engage bone in areas where volume is typically preserved even after years of tooth loss, which means bone grafting can often be avoided.
You may be a candidate if you are missing all or most teeth in an arch, have teeth that are failing and need to be extracted, or are currently wearing dentures and want a fixed alternative. Patients with controlled medical conditions, including diabetes, are generally still eligible.
