- Curved or fused roots — root anatomy that prevents the tooth from being elevated out of the socket in one piece
- Ankylosis — teeth that have fused directly to the surrounding bone
- Failed simple extraction — when a tooth fractures during attempted simple removal and the remaining root must be surgically retrieved

The Surgical Extraction Procedure
Surgical extractions follow a controlled, step-by-step process. While the specifics vary depending on the tooth and its position, the general approach includes:
- Local anaesthetic — the area is thoroughly numbed so you feel pressure but not pain throughout the procedure
- Incision in the gum tissue — a small flap is raised to expose the tooth and surrounding bone
- Bone removal (if needed) — a small amount of bone around the tooth may be carefully removed to provide access
- Tooth sectioning — the tooth is divided into smaller pieces if it cannot be removed whole, reducing the amount of bone removal required
- Tooth removal — each section or the whole tooth is elevated and removed from the socket
- Sutures — the gum is repositioned and stitched closed to promote healing; sutures are typically dissolvable
The entire procedure is usually completed within 30 to 60 minutes depending on complexity. Dr Pham will explain what he is doing at each stage so there are no surprises.
Dr Pham’s Qualifications
Dr Thien Pham graduated with a Bachelor of Dental Science from the University of Melbourne and subsequently completed postgraduate training in oral surgery, and sedation and pain control. These additional qualifications allow him to perform complex surgical extractions at Compass Dental Care that would normally require a referral to an oral surgeon or hospital.

Recovery After a Surgical Extraction

- Soreness: Three to seven days of discomfort is typical, managed with over-the-counter or prescribed pain relief
- Swelling: Peaks on day two to three, then gradually subsides over the following days
- Diet: Soft foods for the first few days — soups, yoghurt, scrambled eggs — gradually returning to normal as healing progresses
- Activity: Rest on the day of surgery; avoid strenuous exercise for 48–72 hours
- Oral hygiene: Gentle rinsing with warm salt water from the day after surgery; avoid brushing directly over the surgical site for the first few days
